چکیده

We describe an unusual case in which an implantable cardioverter-defibrillator failed to defibrillate the patient during testing at implant; rather, an adequate safety margin was achieved only after introduction of a shocking coil into the azygos vein. The azygos vein is an attractive extracardiac target for placement of a shocking coil, as it provides an ideal shock vector. Tools designed for placing epicardial leads via the coronary sinus branches can be adapted to conveniently place the azygos vein coil.

Azygos Vein Lead Implantation For High Defibrillation Thresholds In Implantable Cardioverter Defibrillator Placement

Evaluation of defibrillation threshold is a standard of care during implantation of implantable cardioverter defibrillator. High defibrillation thresholds are often encountered and pose a challenge to electrophysiologists to improve the defibrillation threshold. We describe a case series where defibrillation thresholds were improved after implanting a defibrillation lead in the azygos vein.

Orthostatic increase in defibrillation threshold leading to defibrillation failure and prolonged cardiac arrest in a sitting position: Lessons from a patient’s near-fatal experience

Introduction It has previously been reported that patients with an implantable cardioverter-defibrillator (ICD) may have a slightly higher defibrillation threshold (DFT) in an upright position compared to a supine position. However, the significance of this and the potential impact on clinical practice remains unclear. We report a case of defibrillation failure caused by an increase in DFT in t...

Comparison of single- and dual-coil active pectoral defibrillation lead systems.

OBJECTIVES The purpose of this study was to compare defibrillation thresholds with lead systems consisting of an active left pectoral electrode and either single or dual transvenous coils. BACKGROUND Lead systems that include an active pectoral pulse generator reduce defibrillation thresholds and permit transvenous defibrillation in nearly all patients. A further improvement in defibrillation...

Atrial defibrillation with a transvenous lead: a randomized comparison of active can shocking pathways.

OBJECTIVES The purpose of this study was to compare transvenous atrial defibrillation thresholds with lead configurations consisting of an active left pectoral electrode and either single or dual transvenous coils. BACKGROUND Low atrial defibrillation thresholds are achieved using complex lead systems including coils in the coronary sinus. However, the efficacy of more simple ventricular defi...

Superior Vena Cava Electrode

Background A unipolar defibrillation system using a single right ventricular (RV) electrode and the active shell or "CAN" of the implantable cardioverter-defibrillator itself situated in a left infraclavicular pocket has been shown to be as efficient in defibrillation as an epicardial lead system. The purpose of this study was to determine whether defibrillation efficacy can be improved further...
